Highly skilled Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage plan members throughout Payette County. This per diem position provides both flexibility and meaningful patient interaction in a variety of community settings.

Key Responsibilities:
Conduct comprehensive 45-minute home-based health risk assessments, which may include annual evaluations or post-discharge visits Review and document medical history, ongoing health conditions, and current medications Educate patients on their diagnoses and prescribed therapies, ensuring understanding of treatment regimens Identify care gaps and recommend actionable steps to improve patients health outcomes Visits are strictly assessment-focused with no requirement for treatment planning or medication prescribing
Qualifications:
Active Nurse Practitioner license in Idaho Current CPR (BLS) certification Board certification via AANP or ANCC Completion of an MSN degree from an accredited institution At least six months of recent Nurse Practitioner experience is preferred This rewarding opportunity allows you to serve a diverse Medicare population in home settings across Payette, Fruitland, New Plymouth, and surrounding communities in Payette County.
