Oakstone Hospice is seeking a compassionate, dependable PRN Registered Nurse (RN) to join our hospice team in an on-call capacity . This position is ideal for an experienced nurse who is comfortable working independently, responding to patient and family needs after hours, and providing calm, compassionate care during some of life's most important moments. Position Summary The PRN On-Call RN provides hospice nursing support to patients and families during assigned on-call hours. The nurse will triage calls, provide telephone support when appropriate, make patient visits when needed, and coordinate with the hospice team to ensure patients remain comfortable and well supported. Responsibilities Respond promptly to after-hours calls from hospice patients, families, caregivers, and facilities Provide telephone triage and determine when an in-person nursing visit is needed Make PRN visits for symptom management, changes in condition, pain or comfort concerns, and other urgent patient needs Assess patients and implement appropriate interventions within the hospice plan of care Provide education and emotional support to patients and families Communicate with physicians, pharmacies, facilities, and hospice team members as needed Assist with end-of-life symptom management and death visits/pronouncements as appropriate Complete timely and accurate clinical documentation Provide clear handoff communication to the primary hospice team Maintain a compassionate, professional presence with patients and families during stressful situations Qualifications Current Registered Nurse (RN) license in Idaho Current driver's license, reliable transportation, and automobile insurance Strong clinical assessment and critical-thinking skills Ability to work independently and make sound clinical decisions Excellent communication and documentation skills Dependable and able to respond promptly while on call Hospice, home health, emergency, ICU, oncology, or palliative care experience preferred Hospice experience is a plus, but we are willing to consider the right RN who has a heart for end-of-life care What We're Looking For We are looking for an RN who understands that hospice on-call nursing is about more than responding to a phone call.
