Role Overview:
Travel to patient homes in locations such as Sandpoint, Ponderay, and Priest River within Bonner County Conduct comprehensive annual and post-discharge risk assessments, typically lasting 45 minutes per visit Review past and current medical histories, medical conditions, and medication usage Educate patients on their conditions and medication regimens Identify care gaps and offer recommendations for improved health outcomes Focus exclusively on assessmentno treatment planning or prescribing required
Qualifications:
