Key Responsibilities:
Perform comprehensive home health risk assessments for Medicare Advantage plan members in their homes. Review and evaluate prior and current medical history, medical conditions, and medication usage. Educate members on health conditions and prescribed medications, promoting patient understanding and engagement. Identify gaps in care, offering actionable recommendations to address these areas. Requirements are limited to assessment and education; treatment plans and prescribing activities are not involved.
Qualifications:
Active Nurse Practitioner license in Idaho. Board Certification through AANP or ANCC. Current CPR certification (BLS). Minimum MSN degree from an accredited nursing program. At least 6 months of current NP experience preferred, particularly in home health settings.
