Position Overview Clinical Liaison Horizon Home Health & Hospice is seeking a full‑time Clinical Liaison to represent our home‑health and hospice services throughout the Twin Falls region. This role is ideal for a clinician with an active COTA, LPN, PTA, or RN license who excels at building relationships, educating referral partners, and driving referral growth. Key Responsibilities Liaison Responsibilities Build and maintain strong relationships with referral sources including hospitals, SNFs, clinics, and physician groups. Educate partners on Horizon's home‑health and hospice services, clinical capabilities, and patient‑centered care model. Identify referral opportunities and collaborate with partners to ensure timely, appropriate patient transitions. Leverage clinical expertise (COTA, LPN, PTA, or RN) to communicate patient needs and service benefits effectively. Conduct on‑site visits, presentations, and outreach to strengthen Horizon's presence in the community. Work closely with intake, clinical leadership, and field teams to ensure seamless onboarding of new patients. Track referral trends, outreach activities, and relationship‑building efforts to support strategic growth. Represent Horizon with professionalism, empathy, and CAPLICO‑aligned leadership. Qualifications Licensure Requirements Active COTA, LPN, PTA, or RN license in Idaho (required). Strong communication, relationship‑building, and presentation skills. Experience in home health, hospice, or post‑acute care preferred. Ability to work independently and manage a dynamic outreach schedule. Reliable transportation and valid driver's license.
