Role:

Provides direct patient care in an ambulatory setting, collaborating with providers and care teams to develop and implement personalized care plans aimed at optimizing health outcomes.

Responsibilities:

Assess and evaluate patients' physical, psychosocial, and emotional needs using critical nursing skills. Follow clinical protocols and documentation standards in accordance with compliance guidelines. Support providers by documenting assessments and treatment plans accurately. Educate patients and families with verbal and written instructions, assessing their understanding and addressing questions. Assist in daily patient management, offering oversight and guidance to clinical staff.

Qualifications & Skills:

Education:

Nursing Diploma/Associate's degree in Nursing; Bachelor's degree preferred.

Experience:

2-5 years; 5-10 years preferred for advanced roles.

Licenses & Certifications:

Active RN license, BLS certification.

Competencies:

Strong critical thinking, communication, and patient education skills; ability to work safely and alertly in a healthcare environment.

Additional Information:
