As a Home Health Registered Nurse, you will provide skilled nursing care, education, and ongoing assessment to patients in their homes. You will work closely with patients, families, physicians, and our clinical team to coordinate individualized plans of care.Responsibilities include:
Complete patient admissions, routine nursing visits, and follow-up visits. Travel within your assigned territory to provide skilled nursing care and condition-specific patient education. Assess patients and document observations, progress, and changes in condition. Provide hands-on nursing care according to physician orders and the established plan of care. Collaborate with physicians and the Director of Nursing to develop and update patient treatment plans. Communicate changes in patient condition and progress to the physician and Director of Nursing as appropriate. Educate patients and their families regarding diagnoses, medications, treatments, and ongoing care needs. Work with patients and families to help achieve the best possible outcomes and address identified needs or concerns. Maintain accurate and timely clinical documentation.POSITION REQUIREMENTS
New graduates are encouraged to apply! Graduate of an accredited nursing program. Current Registered Nurse license in the State of Illinois. Current CPR certification. Strong assessment, communication, documentation, and problem-solving skills. Ability to demonstrate required nursing competencies. Basic computer skills necessary to complete electronic clinical documentation. Valid Illinois Driver's License. Current automobile insurance.SERVICE AREA
