Key Responsibilities:

Perform comprehensive annual health risk assessments for Medicare Advantage plan members during home visits Review prior and present medical conditions, including complete medication history Provide personalized education and guidance on chronic conditions and medications to members Identify care gaps and offer valuable recommendations to help resolve health concerns Each assessment lasts approximately 45 minutes, and you may see members for annual exams or post-discharge follow-ups within Rockford and surrounding cities in Winnebago County No duties related to developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Illinois Current BLS (CPR) certification Board Certification via AANP or ANCC Preferred minimum of six months recent Nurse Practitioner experience MSN degree from an accredited nursing program This role offers a competitive opportunity with $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
