Job Description

Role:

RN Case Manager providing compassionate end-of-life care in patients' homes within Bloomington, Pontiac, and nearby areas.

Responsibilities:

Conduct comprehensive, ongoing patient assessments to determine evolving needs. Develop and adjust personalized care plans in collaboration with physicians and care teams. Administer medications and treatments per physician orders. Maintain accurate and timely clinical documentation. Serve as a communication link among families, physicians, and community health partners. Educate patients and caregivers to foster confidence and understanding. Participate in on-call rotations as per team policies.

Qualifications & Skills:

Active RN license in the state of practice. Graduate of an accredited Nursing program. Preferably 1+ years of hospice, home health, or related experience. Strong clinical judgment and ability to work independently. Excellent communication skills with care teams and families. Compassionate approach to end-of-life care.
