Full-Time Productivity:

26 Points Per Week No guessing about productivity expectations. We are transparent from the start. Start of Care (SOC) = 2 points Resumption of Care (ROC) = 1.5 points Recertification = 1.5 points Routine Visit = 1 point Full-Time RNs participate in the weekend on-call rotation approximately once every 4-6 weeks, based on staffing needs. What You'll Do Provide skilled, one-on-one nursing care in patients' homes Complete SOC, ROC, Recertification, routine, and discharge visits Perform comprehensive nursing assessments Develop, implement, and update individualized Plans of Care Provide wound care, medication management, injections, and other skilled nursing treatments Educate patients and caregivers regarding medications, disease management, and home safety Collaborate with physicians, therapists, and the interdisciplinary care team Complete timely and accurate documentation in Axxess EMR Utilize AI-assisted SOC documentation to help streamline the documentation process What We're Looking For Active Illinois Registered Nurse (RN) license Minimum of two years of Med-Surg nursing experience Home Health experience preferred, but not required Strong clinical assessment and documentation skills Excellent communication and time-management skills Valid driver's license and reliable transportation A compassionate, dependable nurse who is comfortable working independently in the field Buffalo Grove Territory Our clinicians are assigned patient territories close to home whenever possible, helping minimize unnecessary drive time and maximize time spent with patients.
