Seeking a Registered Nurse (RN) for ongoing Home Health Case Management in Cary, Illinois, working 40 hours/week during days. Develops therapeutic nurse/patient relationships, assessing and involving families in care planning. Ensures quality, cost-effective care by mobilizing resources, adhering to standards, and participating in quality improvement. Provides patient and family education, tailoring approaches to learning needs, and promotes staff development. Uses nursing processes to recognize changes, make clinical decisions, and coordinate interdisciplinary care and discharge plans. Demonstrates leadership, teamwork, communication, and maintains professional growth with required certifications. Must hold an active Illinois RN license, CPR certification, and have a minimum of one year of medical-surgical experience.
