Primary Responsibilities:

Complete comprehensive home health risk assessments, spending approximately 45 minutes per visit Review prior and current medical histories, chronic conditions, and medications Educate patients regarding diagnoses, medication regimens, and best practices for ongoing care Identify and address gaps in care, making recommendations for further follow-up as needed Visits may include annual health assessments or post-discharge follow-ups No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Illinois Board Certification via the AANP or ANCC Current Basic Life Support (CPR/BLS) certification Master of Science in Nursing (MSN) from an accredited institution At least six months of current nurse practitioner experience is preferred Gain competitive compensation, with each evaluation earning $80$90, plus a mileage stipend and additional pay for administrative time.
