2866208 Private Duty Nurse needed with experience in Advanced Dementia. Home Care client needs a Licensed Practical Nurse or Registered Nurse for his ongoing schedule.

Shift Need:

Wednesdays and Thursdays from 7am-1pm; Fridays 7am-7pm (24 Hours per Week) (Additional client shifts available if requested)

Pay:

$38-40/

Nurse Job Duties:

Perform care as defined by client's individual Plan of Care and within scope of state's nurse practice act. Monitor patient's condition and notify appropriate personnel of any changes. Provide those services requiring substantial and specialized nursing skill. Assures proper maintenance of clinical records in compliance with local, state and federal laws. Counsels the client and family in meeting nursing and related needs.

Position Requirements Nurse Qualifications:

Current and unrestricted RN or LPN license in state (Illinois) for which you are applying. Home Health Experience Minimum 2 years experience as a nurse. Proof of eligibility to work in the US. Reliable car / auto insurance. Clean background and criminal record. #IND456
