We are seeking a dependable and experienced Registered Nurse (RN) to provide Start of Care (SOC) assessments and supervisory visits for children receiving home nursing services through
UIC-DSCC
(University of Illinois Chicago Division of Specialized Care for Children) . The RN will conduct comprehensive in home assessments, review the patient's plan of care, supervise nursing services, provide caregiver education, and ensure that care is delivered safely and in accordance with physician orders, UIC-DSCC requirements, and agency policies. Responsibilities Complete Start of Care (SOC) visits for new pediatric home care patients. Perform comprehensive head to toe nursing assessments. Review and implement the physician approved plan of care. Assess the child's medical condition, medications, treatments, and equipment. Complete required SOC documentation accurately and timely. Conduct supervisory visits for patients receiving ongoing home nursing services. Evaluate the quality and appropriateness of nursing care being provided. Observe and assess assigned nurses/caregivers during supervisory visits. Review medication administration, treatments, infection-control practices, safety, and documentation. Monitor for changes in the patient's condition and communicate significant findings to the physician, care team, agency, and appropriate parties. Provide education and support to parents/caregivers. Ensure documentation meets agency, UIC-DSCC, and applicable regulatory requirements. Participate in care coordination and follow up as needed. Maintain patient confidentiality and professional standards at all times. Qualifications Current, unrestricted Illinois RN license . Pediatric/home health nursing experience preferred. Experience with medically complex children is highly preferred. Familiarity with
UIC-DSCC
cases and requirements is preferred. Strong assessment, communication, and documentation skills. Ability to work independently in the patient's home. Reliable transportation and ability to travel to assigned patient homes. Current CPR/BLS certification. Ability to complete documentation electronically. Strong understanding of infection prevention, patient safety, and pediatric home-care practices.
