Deliver high-quality patient assessments in a meaningful, community-based role as a Nurse Practitioner specializing in home health risk evaluations. In this per diem position, youll support Medicare Advantage plan members throughout Ogle County, including areas such as Rochelle, Byron, and Oregon. Each assessment averages about 45 minutes, focusing on annual health reviews or post-discharge check-inshelping patients with education, care recommendations, and comprehensive record updates.

Key Responsibilities:

Complete in-home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical histories, chronic conditions, and medication lists Educate patients about their health issues and medications, enhancing their understanding of their care Identify gaps in care and deliver practical recommendations to promote wellness and prevent hospital readmission

Desired Qualifications:

Active Nurse Practitioner license in Illinois Board Certification via AANP or ANCC Current CPR (BLS) certification MSN from an accredited nursing program Minimum of 6 months Nurse Practitioner experience preferred Youll earn $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time, allowing you to maximize your earnings while supporting patient health where it matters most.
