2989949751-2 Apply for this job now Job Description Humana is seeking a compassionate Field Care Manager Nurse to coordinate and deliver high-quality care to members in their homes and communities. In this role, you'll conduct in-person assessments, develop individualized care plans, and collaborate with physicians, social workers, and caregivers to close care gaps and improve outcomes. You will educate members and families on conditions, medications, and preventive care, while monitoring progress and adjusting plans as needs change. You'll leverage Humana's resources, data, and technology to support holistic, person-centered care and connect members with community services that enhance health and well-being. Responsibilities Conduct comprehensive in-home and community-based health assessments for members Develop, implement, and update individualized care plans based on clinical findings Coordinate care with physicians, social workers, and interdisciplinary team members Educate members and caregivers on conditions, medications, and self-management Monitor member progress, identify changes in status, and adjust care accordingly Close care gaps through proactive outreach, follow-up, and preventive care support Document all assessments, care plans, and interventions in electronic health records Connect members with community resources and social support services Advocate for members' needs to improve access, quality, and continuity of care Use data and reporting tools to identify risk, prioritize outreach, and track outcomes Required Skills Clinical assessment Care planning Case management Patient and family education Care coordination Electronic health records (EHR) Chronic disease management Medication management Community resource navigation Data-informed decision making
