Job Description

Transitions Care is seeking a Registered Nurse Admissions RN to guide patients and families through the hospice admission process. In this role, you will complete comprehensive assessments, initiate individualized plans of care, and provide education on hospice services, symptom management, and medications. You will collaborate closely with an interdisciplinary team to coordinate smooth transitions from hospitals, facilities, and homes, ensuring dignified, patient- and family-centered end-of-life care. Responsibilities Conduct comprehensive hospice admission assessments, including physical, psychosocial, and spiritual evaluations Develop, implement, and update individualized hospice plans of care in collaboration with the interdisciplinary team Educate patients and families on hospice philosophy, symptom management, medications, and available services Coordinate transitions of care from hospitals, facilities, and homes into hospice services Administer medications and treatments to manage pain and distressing symptoms per physician orders and hospice protocols Communicate changes in patient condition to physicians and team members and adjust care plans as needed Document assessments, interventions, and education accurately and timely in the electronic health record Participate in interdisciplinary team meetings and case conferences to ensure coordinated, holistic care Provide emotional support and guidance to patients and families during end-of-life decision-making Ensure compliance with hospice regulations, safety standards, and organizational policies Required Skills Hospice and palliative nursing Comprehensive patient assessment Care planning and coordination End-of-life symptom management IV and medication administration Electronic health records (EHR) documentation Patient and family education Interdisciplinary team collaboration Regulatory and hospice compliance Crisis and emotional support
