Responsibilities Provide skilled nursing care to patients in their homes Complete patient assessments and develop individualized care plans Administer medications and treatments as ordered by the physician Monitor patient condition and communicate changes to the interdisciplinary team Educate patients and caregivers on disease management and treatment plans Maintain accurate and timely clinical documentation Collaborate with physicians, therapists, and other healthcare professionals to ensure quality patient care Qualifications Active Illinois RN or LPN License Minimum one year of nursing experience preferred Experience in home health, hospice, hospital, skilled nursing, rehabilitation, or long-term care preferred Knowledge of Medicare and home health regulations is a plus Experience with Axxess and WellSky (Kinnser) preferred Valid driver's license, reliable transportation, and auto insurance Excellent communication and organizational skills What We Offer Competitive Pay Per Visit Rates Flexible Scheduling PRN Opportunities Supportive Clinical Team Opportunity for
