A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Hour Available Monday - Friday 11:00am - 7:00pm 1055 E Grand Ave Lindenhurst, IL 60046 Position Summary The Admission Nurse is responsible for coordinating and completing the admission process for new residents entering the skilled nursing community. This role ensures accurate clinical assessments, physician order implementation, medication reconciliation, and effective communication with residents, families, and providers. This role ensures all admissions meet regulatory and ministry requirements. Essential Functions, Key Duties, & Responsibilities Complete comprehensive nursing admission assessments upon arrival. Review hospital discharge documentation and transfer records. Verify physician orders and ensure timely implementation. Perform medication reconciliation and communicate discrepancies to the attending physician or medical provider. Assess residents for immediate nursing needs, including pain management, fall risk, skin integrity, nutrition, and infection prevention. Initiate individualized care plans. Complete admission documentation in the electronic medical record (EMR) accurately and within required timeframes. Ensure all required regulatory and facility admission documentation is completed. Communicate admission information to nursing staff, therapy, dietary, pharmacy, social services, activities, and other departments. Educate residents and families regarding community policies, routines, resident rights, and available services. Follow infection prevention and control protocols. Respond appropriately to emergencies and changes in resident condition. Collaborate with nursing leadership, therapy, social services, dietary, life enrichment, and other departments to ensure a seamless admission process. Address questions and concerns with professionalism, empathy, and responsiveness. Promote a welcoming, resident-centered experience throughout the transition. Ensure compliance with all federal, state, and local regulations governing admissions. Maintain accurate, confidential, and timely documentation in the electronic health record. Verify completion of required clinical forms, consents, and physician documentation at time of admission. Perform other duties as assigned. Comply with all policies and procedures and any updates.
Position Requirements Education :
A nursing degree from an accredited college or university preferred
Licenses/Certifications:
Current unrestricted license as Registered Nurse (RN) in practicing state
Experience :
2+ years of Nursing experience preferred
