Responsibilities include comprehensive assessments, ventilator and trach management, IV therapy, wound care, and telemetry monitoring. The RN will collaborate with physicians and the care team, educate patients and families, and maintain accurate EMR documentation. Responsibilities Provide comprehensive nursing assessments and ongoing monitoring for LTAC patients Manage ventilators, tracheostomies, and complex respiratory needs Administer medications, IV therapies, and manage central lines per provider orders Perform wound care, including complex and chronic wound management Monitor telemetry and respond quickly to changes in patient condition Document care, assessments, and interventions accurately in the EMRCollaborate with physicians, therapists, and the interdisciplinary team on plans of care Educate patients and families on treatments, discharge plans, and self-care Ensure adherence to infection control, safety, and regulatory standards Support a patient-centered, clinician-focused culture and high-quality outcomes Required Skills Long-term acute care nursing Ventilator and tracheostomy management Wound care Medication administration IV therapy and central line care Telemetry monitoring Care planning and documentation (EMR) Infection control and patient safety Patient and family education Interdisciplinary team collaboration
