Overview Join our compassionate healthcare team as a PRN Hospice Weekend Nurse, where your expertise will provide vital comfort and care to patients during weekends. This flexible role offers the opportunity to make a meaningful difference in the lives of individuals facing end-of-life journeys, ensuring they receive respectful, personalized care in their familiar environments. As a key member of our hospice team, you'll deliver skilled nursing services with energy, empathy, and professionalism, supporting patients and their families during critical moments. Responsibilities Deliver comprehensive nursing care to hospice patients during weekend shifts, including assessments, medication administration, and symptom management. Monitor patient vital signs and conduct physical examinations to evaluate health status and adjust care plans accordingly. Manage catheter care, and perform sterile procedures following best practices for infection control. Collaborate with interdisciplinary teams to coordinate patient care, discharge planning, and family education. Document all patient interactions accurately using electronic health record (EHR) systems while maintaining strict compliance with HIPAA and confidentiality policies. Provide emotional support and education to patients and families about disease progression, medication management, and comfort measures. Respond promptly to urgent patient needs through triage protocols and emergency procedures, including airway management and acute pain control. Complete routine visits and new admissions Experience Proven experience working with hospice or palliative care patients in settings such as long-term care facilities, hospitals, or home health environments Critical care experience in ICU or emergency room settings is highly valued for managing complex symptoms and acute situations. Familiarity with ICD coding (ICD-9/10) for accurate documentation of diagnoses. Knowledge of geriatrics, dementia care—including Alzheimer's and memory care—and working with individuals with developmental disabilities or behavioral health needs. Ability to perform patient assessments involving vital signs, physiology knowledge, medication administration, and symptom management in accordance with long-term care regulations.
