This role ensures a positive membership experience by coordinating clinical care, assisting with patient education, managing chronic conditions, and supporting front-office operations when needed. The RN/LPN promotes MDVIP's values by fostering long-term patient relationships, contributing to membership growth/retention, and maintaining compliance with clinical standards. Essential Duties & Responsibilities Clinical Care & Patient Support Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice. Prepare patients for examinations and procedures, ensuring comfort and understanding. Perform patient assessments and document clinical findings accurately in the EMR (AthenaNet). Coordinate follow-up care, referrals, and preventive screenings. Educate patients on disease management, medication use, wellness strategies, and preventive care. Membership Experience & Retention Assist physician in delivering personalized care aligned with MDVIP's concierge model. Support membership retention by building relationships with patients and providing exceptional service. Participate in wellness initiatives, and advanced patient services. Administrative & Operational Support Assist with scheduling, phone triage, and patient communication when needed. Verify patient insurance, demographics, and eligibility. Participate in quality improvement initiatives and compliance audits. Maintain accurate logs for vaccines, CLIA-waived tests, and other regulatory requirements. Team Collaboration Work closely with physicians, medical assistants, and practice coordinators to ensure seamless care delivery. Assist with onboarding and mentoring of new staff as needed.

Education, Experience, and Knowledge Requirements Education :

Graduate of an accredited nursing program; BSN preferred.

Licensure :

Active RN or LPN license in the state of Illinois (in good standing).

Experience :

Minimum 2 years of clinical nursing experience, preferably in primary care, chronic disease management, concierge medicine, or outpatient practice.

Knowledge/Skills :

Proficiency with EMR systems (AthenaNet experience preferred). Strong clinical assessment, triage, and documentation skills. Excellent communication and patient service orientation. Ability to manage multiple priorities in a fast-paced environment. Knowledge of regulatory standards (HIPAA, OSHA, CLIA, CCM billing requirements).
