• Provide management and delivery of patient care plans
• Support patients in Mount Carmel and surrounding areas
• Reward the accomplishment of key achievements during the first year Schedule and Shift Details
• : 7 days on followed by 7 days off:

Mon-Thurs:

5P to 8A; Friday

• Sunday:

  8A to 8A (would more the less be a on call position during the weekends)
• (2) holidays per year
• 1 summer & 1 winter; 3 floating holidays Qualifications Required Associate degree or higher Hospice (2+ years) 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR TruChart EHR Valid Registered Nurse (RN) in Illinois (IL) Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
