Job Overview The Home Health Visiting Registered Nurse (RN) delivers skilled nursing care to patients in their homes as prescribed by the physician and in accordance with applicable laws, regulations, and the organization's standards of care. The RN coordinates care with interdisciplinary team members and promotes patient independence and wellness in a home setting. Duties Conduct initial and ongoing assessments of patient needs and conditions. Develop and revise individualized care plans in collaboration with patients, caregivers, physicians, and other healthcare professionals. Provide skilled nursing care such as wound care, IV therapy, medication administration, catheter care, and chronic disease management. Educate patients and caregivers on disease processes, medication use, and preventative health measures. Monitor patient progress and modify care plans as necessary. Document all care and observations in accordance with agency policy and regulatory requirements. Communicate with the physician regarding changes in the patient's condition or care needs. Coordinate with home health aides, therapists, and other clinical staff to ensure high-quality patient outcomes. Participate in case conferences and team meetings. Comply with infection control, safety, and HIPAA guidelines.

Qualifications:

Licensure:

Active and unrestricted RN/LPN license in the state of practice

Education:

Graduate of an accredited School of Nursing (Associate's or Bachelor's degree in Nursing)

Experience:

Minimum 1 year of clinical nursing experience (home health experience preferred)

Certifications:

CPR certification required; additional certifications a plus (e.g., IV therapy, wound care)

Driver's License:

Valid driver's license and reliable transportation required
