Hospice Registered Nurse (RN) Case Manager – Full-Time | Northbrook IL JourneyCare Hospice Tired of high caseloads, frequent weekend rotations, and feeling stretched too thin? At JourneyCare Hospice, we do things differently. We prioritize manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families. We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area.
Schedule & Territory Territory:
Northbrook, IL and surrounding communities. Monday-Friday, 8:30 AM – 5:00 PM. No On-Call One weekend rotation every 12 weeks and 1 summer and 1 winter holiday Compensation & Benefits $80,000 – $97,000 annually (based on experience) 401(k) with company match Medical, dental, and vision insurance (effective first of the month after hire) Generous PTO and paid holidays Mileage reimbursement Tuition reimbursement Hospice certification support with salary increase Merit-based raises Employee referral bonuses and discount programs Key Responsibilities Manage a dedicated hospice caseload as the primary RN Case Manager Perform patient assessments, admissions, and individualized care planning Deliver skilled nursing care focused on comfort and dignity Coordinate care with physicians and interdisciplinary team members Conduct routine visits and adjust care plans as needed Provide end-of-life support, including family education and pronouncement Maintain timely, accurate documentation Supervise Home Health Aides in compliance with regulations Qualifications Graduate of an accredited RN program Active Illinois RN license Current CPR and First Aid certification Hospice or home health experience preferred Strong clinical judgment and communication skills Valid driver's license and proof of insurance If you're looking for a hospice RN role where you can provide meaningful care without burnout, this is the opportunity for you.
