A Public or Community Health Nurse provides health care to community members and addresses community health issues ranging from infectious diseases to substance abuse, focusing on prevention and education as well as treatment. Works with a team of health care professionals; may work in a clinic, and travel within a community or between different communities. May serve as the primary health care provider in underserved communities.
Position Summary The Community Health Navigation Program Lead serves as the clinical lead for POC's Community Health Initiative, improving access to healthcare for underserved residents throughout Chicago's northwest suburbs. The Program Lead works alongside Community Health Workers (CHWs), Health Navigators, healthcare providers, and community partners to improve health literacy, increase preventive care, reduce barriers to healthcare, and connect residents to primary care and community resources. This position focuses on chronic disease prevention and management, health screenings, care coordination, and addressing barriers to social determinants of health through culturally responsive, community-based services. Essential Responsibilities 1. Clinical Care & Education Provide health education on chronic disease prevention and management. Conduct and facilitate community health screenings including blood pressure, glucose, BMI, and other approved screenings. Interpret screening results and provide education on next steps. Assist participants in understanding medical diagnoses, medications, and treatment. Develop culturally appropriate health education materials. 2. Care Coordination Connect participants with primary care providers and specialists. Coordinate follow-up appointments after hospitalizations or emergency department visits. Work with hospitals and healthcare providers to ensure continuity of care. Assist with referrals for behavioral health, specialty care, and preventive services. 3. Program Leadership Provide clinical supervision and guidance to Community Health Workers. Assist in developing and delivering Community Health Navigator training. Handle difficult cases and prioritization of work of CHWs and navigators Coordinate health fairs, educational workshops, and outreach events. Maintain relationships with healthcare partners including Endeavor Health, Ascension, Cook County Department of Public Health, and others. 4. Data & Evaluation Maintain accurate client documentation. Track screenings, referrals, outcomes, and participant progress. Assist with grant reporting and program evaluation. Qualifications Registered Nurse (RN) licensed in Illinois (BSN preferred) or equivalent public health professional Minimum two years of community, public health, or care coordination experience Demonstrated ability working effectively with culturally diverse populations Knowledge of chronic disease management Computer/tech knowledge (Google Suite) Bilingual English/Spanish preferred Flexibility of schedule and ability travel through the Chicagoland area as necessary; reliable transportation required Previous managerial experience Excellent communication and relationship-building skills
