We are looking for an experienced Hospice Admissions RN who brings confidence, compassion, and strong clinical judgment to one of the most important moments in a family's hospice journey. This is more than completing an admission. You will often be the first nurse a patient and family meets. You will help them understand hospice, answer difficult questions, create trust, and make sure their transition into care feels organized, personal, and supported from the start. What You'll Do As a Hospice Admissions RN, you will complete admissions in patient homes, hospitals, skilled nursing facilities, assisted living communities, and other care settings throughout Deerfield, Skokie, North Chicago, Northwest Chicago, and surrounding communities . You will assess patients for hospice eligibility, complete clinical documentation, coordinate physician orders, communicate with the interdisciplinary team, and educate patients and families about hospice services, goals of care, and what to expect next. Most importantly, you will help families feel informed, respected, and supported during a difficult time.
