We're seeking a detail-oriented RN with a passion for resident-centered care to join our team as an MDS / Care Plan Coordinator. As a key member of our interdisciplinary care team, you'll be responsible for ensuring our residents receive care that is both clinically sound and deeply personalized. About the
Role:
As our MDS / Care Plan Coordinator, you'll be responsible for: Coordinating timely and accurate MDS assessments Leading interdisciplinary Care Plan meetings Monitoring EMR documentation for accuracy and compliance Conducting in-services and training on care planning/documentation best practices Collaborating with physicians, therapy teams, and department leaders Ensuring compliance with state and federal regulations
What We're Looking For:
Current Illinois RN License MDS experience AANAC certification (Preferred) Proficiency in EMR systems and documentation Excellent leadership, communication, and organizational skills A compassionate, resident-focused mindset Why Choose Haven?
