Job Description

Elevate patient care with meaningful, direct-impact work as a Nurse Practitioner in a dynamic, per diem home health environment. Utilizing your clinical expertise, youll conduct risk assessments for Medicare Advantage members in Peoria County, ensuring a supportive bridge for patients between care settings. With flexibility in scheduling and the ability to design your own shifts, this is an ideal opportunity for professionals who seek a highly adaptable and rewarding role.
Schedule:
Flexible shifts available Monday through Sunday Design your own work schedule Choose minimum weekly or bi-weekly hourly commitments
Core Responsibilities:
Complete comprehensive annual home health risk assessments for Medicare Advantage plan members Review and evaluate both past and current medical and medication histories Provide health education to members about conditions and prescribed medications Identify care gaps and offer recommendations to address them Conduct appointments in various locations across Peoria County, including Peoria, Chillicothe, and Bartonville No prescribing or treatment plan creation required
Essential Qualifications:
Current Nurse Practitioner license in the state of Illinois Active CPR certification (BLS) Board certification via AANP or ANCC At least 6 months of recent nurse practitioner experience (preferred) MSN from an accredited nursing institution
