Transitions Care is seeking a compassionate Registered Nurse Admissions to welcome patients into our hospice program. In this role, you will complete comprehensive admission assessments, educate patients and families on hospice services, and initiate individualized plans of care. You'll collaborate closely with an interdisciplinary team to ensure a smooth transition, effective symptom management, and exceptional end-of-life support. Responsibilities Conduct comprehensive hospice admission assessments for new patients in home and facility settings Explain hospice services, philosophy, and care plans to patients and families with clarity and compassion Develop initial individualized plans of care in collaboration with the interdisciplinary team Obtain and document consents, histories, medications, and advance directives accurately Coordinate with physicians, social workers, chaplains, and aides to ensure smooth start of services Evaluate symptom management needs and initiate appropriate nursing interventions Provide patient and family education on disease process, medications, and comfort measures Ensure timely, accurate electronic documentation in accordance with regulatory and agency standards Participate in on-call rotation or flexible scheduling to support timely admissions Uphold infection control, safety, and quality standards while representing Transitions Care's mission and values Required Skills Hospice nursing Admission assessment Patient and family education Care planning Electronic health record (EHR) documentation Pain and symptom management Interdisciplinary care coordination End-of-life care best practices Infection control and safety Time management and prioritization
