Responsibilities Manage a caseload of patients in the field, coordinating care across providers and services Conduct in-home or community-based assessments to evaluate medical, social, and behavioral needs Develop and update individualized care plans in collaboration with patients, families, and interdisciplinary teams Provide patient and caregiver education on conditions, medications, and self-management strategies Monitor patient progress, identify gaps in care, and intervene to prevent avoidable hospitalizations Communicate regularly with physicians, specialists, and payors to align on care goals and authorizations Document all assessments, interventions, and outcomes accurately in electronic health records Ensure compliance with clinical protocols, regulatory requirements, and CVS Health quality standards Advocate for patient access to community resources, benefits, and support services Participate in ongoing training, performance improvement, and cross-functional collaboration Required Skills Case management Clinical assessment Care planning RN clinical skills Chronic disease management Patient and caregiver education Utilization management Electronic health records (EHR) documentation Care coordination across interdisciplinary teams Knowledge of community health resources
