Key Responsibilities:
Perform comprehensive annual or post-discharge risk assessments in patients homes throughout Adams County (including Quincy, Camp Point, Payson) Review and evaluate prior/current medical history, health conditions, and medication usage Deliver clear education to patients regarding medical conditions and medications Identify care gaps and provide action-oriented recommendations, supporting optimal patient outcomes No requirement for creating treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Illinois Current CPR certification (BLS) Board Certification through AANP or ANCC At least 6 months of recent NP experience preferred MSN degree from an accredited nursing program This role offers competitive compensation of $80-$90 per completed evaluation along with a mileage stipend and administrative time.
