Submittal Requirements:

Active RN State License for IL Active BLS recent 2 years' experience in

LTC & SNF

Experience with All scripts Charting system Responsibilities Provide safe, compassionate nursing care to post-acute patients Assess and monitor patients' medical conditions and progress Administer medications and treatments according to orders Perform skilled nursing procedures and wound care Collaborate with physicians, therapists, and other members of the healthcare team Assist patients with progressing toward greater independence Educate patients and families regarding care, medications, and discharge needs Complete accurate and timely documentation in Allscripts Participate in admission and discharge processes Provide compassionate end-of-life care when appropriate Patient Population Our SNU cares for patients with a wide range of post-surgical and chronic medical conditions, including: Hip fractures and orthopedic injuries Joint replacements Pneumonia Sepsis COPD exacerbations Congestive heart failure (CHF) Diabetes Renal failure Cerebrovascular accidents (CVA/stroke) Cancer Post-operative recovery

Pay:

$2,400.00 - $2,500.00 per week
