Key Responsibilities:
Complete thorough annual home health risk assessments for Medicare Advantage members Review prior and current medical history, conditions, and medication usage Educate members on their diagnoses, conditions, and medications Identify gaps in care and make practical recommendations to close them Focus exclusively on assessment and educationno prescribing or treatment plan creation required
Qualifications:
Active Nurse Practitioner license in Illinois Current BLS/CPR certification Board certification via AANP or ANCC Minimum of 6 months recent experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution Youll receive payment at $80$90 per evaluation, with an additional stipend for mileage and compensation for administrative time.
