This role is ideal for nurses who are passionate about patient-centered care and interested in specialty nursing. You'll assess and triage patient needs, coordinate care, provide education, and support patients. Provide telephone triage and guide patients to the appropriate level of care or follow-up. Communicate effectively with providers and the clinical team to ensure timely, appropriate care. Educate patients and caregivers on conditions, treatments, medications, procedures, and home care. Accurately document patient interactions, assessments, and care plans in the electronic health record (EHR). Track patient needs and coordinate follow-up care to promote continuity and positive outcomes. Qualifications + + Registered Nurse (RN) - Active/Unencumbered RN license in state of practice or compact license including state of practice. Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred. Strong communication, assessment, and patient education skills
