Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes Review and document patients medical history, current health status, and medication regimen Provide patient education regarding diagnoses, treatment options, and medication management Identify opportunities to close gaps in care and deliver recommendations to enhance patient outcomes Focus exclusively on assessmentno treatment plan development or medication prescribing necessary

Qualifications:

Active Nurse Practitioner license in the state of Illinois Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR certification (BLS) At least six months of nurse practitioner experience strongly preferred This position offers a compensation structure of $80$90 per completed evaluation, with added mileage stipend and administrative time to ensure your dedication is recognized.
