Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice RN Case Manager to join our team in Saint Charles, IL. In this meaningful role, you'll be at the heart of delivering exceptional end-of-life care, guiding patients and their families through one of life's most important journeys. Responsibilities:

Manage a caseload of hospice patients, conducting comprehensive assessments and developing individualized care plans Perform skilled nursing visits in patient homes, skilled nursing facilities, and other care settings throughout the Saint Charles area Coordinate interdisciplinary team (IDT) care, collaborating with physicians, social workers, chaplains, and aides Attend every-other-week Interdisciplinary Team (IDT) meetings (in person or via phone/video) to review patient plans of care, evaluate progress, and collaborate on care coordination Monitor and manage pain and symptom control, adjusting care plans as patient needs evolve Educate patients and families on disease progression, comfort care, and end-of-life planning Maintain accurate and timely clinical documentation in compliance with hospice regulations and Medicare guidelines Participate in the scheduled on-call rotation to provide coverage for urgent patient and family needs after hours and on weekends

Requirements:

Active RN license in the state of Illinois, in good standing Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred Strong knowledge of Medicare hospice regulations and care standards Excellent clinical assessment, pain management, and symptom control skills Exceptional communication and interpersonal skills with patients, families, and care teams Ability to work independently, manage time effectively, and prioritize a dynamic caseload Valid driver's license and reliable transportation for field-based visits
