POSITION SUMMARY
The Maternal Child Health Nurse (MCH Nurse)/OB Nurse Liaison serves as a critical bridge between prenatal care, medication-assisted (MAT), behavioral health, and MAR recovery services. This role ensures that pregnant women and mothers with dependent children receive integrated, coordinated care throughout pregnancy, postpartum, and early parenting conducts on-site, home visits, and Mobile Clinic visits in the community.
ESSENTIAL DUTIES & RESPONSIBILITIES
Serve as the primary clinical liaison between healthcare providers, behavioral health specialists, case managers, and community resources. Monitor and support maternal and child health, including prenatal, perinatal, and postpartum care. Coordinate and track participation in medication-assisted treatment (MAT) programs, substance use recovery services, and related behavioral health care. Conduct health assessments, provide education, and reinforce adherence to treatment plans. Develop individualized care plans that integrate medical, behavioral, and social supports. Participate in multidisciplinary case conferences and care coordination meetings. Identify and address barriers to care, including transportation, childcare, and social determinants of health. Maintain accurate documentation of clinical services and patient interactions in compliance with organizational and funder requirements.
REQUIRED QUALIFICATIONS
Registered Nurse (RN) with current state licensure; certification in maternal-child health or obstetrics preferred. Experience in prenatal, postpartum, or pediatric nursing; familiarity with MAT and substance use treatment preferred. Knowledge of trauma-informed, family-centered, and culturally competent care. Strong communication, clinical assessment, and care coordination skills. Ability to work collaboratively in a multidisciplinary team and liaise effectively with community partners.
COMPETENCIES
Strong understanding of perinatal health, maternal-child outcomes, and behavioral health integration. Ability to advocate for patients while navigating complex healthcare and social service systems. Skilled in-patient education, counseling, and motivational interviewing techniques.
