2989949573-2 Apply for this job now Job Description Humana is seeking a Field Care Manager Nurse to coordinate and deliver high-quality care to members in their homes and community settings. In this role, you'll conduct comprehensive assessments, develop person-centered care plans, and collaborate with physicians, social workers, and caregivers to improve health outcomes. You will educate members on conditions, medications, and preventive care while monitoring progress and adjusting plans as needed. This role blends autonomous field work with virtual collaboration, supporting Humana's mission to provide accessible, personalized, and value-based healthcare services. Responsibilities Conduct in-home and community-based nursing assessments for assigned members Develop and update individualized, evidence-based care plans Coordinate care with physicians, social workers, and other interdisciplinary team members Provide education on disease management, medications, and preventive care Monitor member progress, track outcomes, and adjust care plans as needed Advocate for members' needs, removing barriers to care and services Document all encounters and care activities in electronic health records accurately and timely Identify high-risk members and implement interventions to reduce hospitalizations and ER visits Support transitions of care from hospital to home or other settings Comply with clinical, regulatory, and quality standards and participate in performance improvement initiatives Required Skills Care management Nursing assessment Care planning Chronic disease management Utilization management Electronic health records (EHR) Interdisciplinary care coordination Patient education Regulatory and compliance knowledge (Medicare/Medicaid) Data and outcomes tracking
