Home Health Organization located in the Chicago Land area. We have an opening for a Home Health RN (Registered Nurse) to conduct patient in home visits in the Tinley Park area South Suburbs Approx 20 patient visits per week

Home Health Nurse RN Responsibilities:

Completing nursing process in delivery of patient care which includes: assessment, planning, implementation, and evaluation of patient outcomes. Total oversight of patient from admission to discharge. Developing plan of care based on findings. Serving as patient's advocate and maintaining ongoing communication with physician. Timely completion of all skilled nursing documentation

Qualifications:

Current Illinois State RN License Valid Drivers License and Reliable Vehicle MINIMUM of 2 years as an RN Strong clinical skills to work in patients homes independently. Knowing when to ask for support from the Director of Nursing Must be RELIABLE and
