We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area.

Schedule & Territory Territory:

Vernon Hills, 60061 and surrounding communities. Monday-Friday, 8:30 AM - 5:00 PM. No On-Call. One weekend rotation every 12 weeks.
