Key Responsibilities:

Complete comprehensive annual and post-discharge home health risk assessments in patient homes Review and document prior and current medical history, medical conditions, and medication usage Educate members about their health conditions and medications, empowering them for better outcomes Identify care gaps and provide actionable recommendations to coordinate additional support as needed Typical assessment duration is about 45 minutes per patient

Qualifications:

Active Nurse Practitioner license in Illinois Board Certification by AANP or ANCC Completion of an accredited MSN program Current BLS (CPR) certification Six months or more of recent NP experience (preferred) Enjoy compensation at $80$90 per evaluation, including a mileage stipend and paid administrative time for the vital work performed in Peoria, Chillicothe, Bartonville, and other Peoria County communities.
