Cass Ave Westmont, IL 60559 ph. 708-412-4554 https://www.alternativehhcare.com Summary Seeking attentive RN and LPN Nurses to visit and provide nursing care to patients in their homes located in Cook, DuPage counties and surrounding area on an intermittent basis. Employment status is Full Time, Per Diem or Part tIme. Accept and schedule patient's when it is convenient for you. Great opportunity for nurses looking to pick up a side job for extra money. Pay rates are per visit and scales up from there depending on travel and visit type. Duties

• Patient assessment and observation in patient's home environment
• Administration and monitoring of prescribed medications, nursing skills, treatments, and/or therapies.
• Follow designed Plan of Care
• Communication with other members of the patients' multi-disciplinary team
• Prepare clinical notes and update primary Physician
• Health promotion and teaching

Requirements:

• Graduate of an accredited school of nursing; RN or LPN Program
• Current and unrestricted licensure in IL
• Valid driver's license and auto insurance
• Health/work clearance letter signed by MD
• Proof of negative tuberculosis
• Covid-19 vaccinated or must be willing to submit weekly negative covid-19 test results.
• CPR certificated
• Be able to work and travel independently.
• Competency with various nursing skills: wound care, bladder care, bowel care, medication management, disease management, etc.
• Proficiency with basic computer operations such as typing, internet, and email
• Excellent observation, verbal, and written skills
• At least a year of clinical patient care experience in a hospital or other clinical setting
• Preferred at least 1 year of prior home health visiting experience.
• Preferred experience in any home health electronic medical record (EMR) system such as Kinnser/Wellsky, Axxess, DataSoft Logic, Homecare Homebase, etc.
