Key Responsibilities:
Conduct thorough home health risk assessmentseach lasting approximately 45 minutesin patients homes Assess medical histories, current conditions, and medication use to gain a holistic understanding of patient needs Educate patients regarding conditions, medications, and preventive steps to promote optimal health Identify care gaps and offer targeted recommendations to improve outcomes Visits may include annual health reviews or post-discharge follow-ups No ongoing care management, treatment planning, or prescribing required, allowing you to focus on impactful, episode-based assessments
Qualifications:
Active Nurse Practitioner License in Indiana Board certification via AANP or ANCC Current CPR (BLS) certification Completion of an MSN degree from an accredited nursing school At least six months of recent Nurse Practitioner experience preferred, especially in a home health or related setting This per diem role provides a pay structure of $80-90 per evaluation, along with a mileage stipend and compensation for administrative time.
