Key Responsibilities:

Perform comprehensive annual health risk assessments for Medicare Advantage members in their homes Collect and evaluate both past and current medical history and conditions, as well as medication usage Educate members on current health conditions and prescribed medications Pinpoint care gaps and recommend necessary follow-ups No treatment planning or prescribing involved

Qualifications:

Active Nurse Practitioner license in Indiana Board Certification from AANP or ANCC Graduate of an accredited MSN program Current BLS (CPR) certification At least 6 months of Nurse Practitioner experience preferred This per diem opportunity provides competitive compensation$80-$90 per completed evaluationwith additional mileage reimbursement and administrative time included.
