Job Overview We are seeking a dedicated and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, including experience with various clinical settings such as long-term care, assisted living, and acute care environments. This role offers an opportunity to make a meaningful difference in the lives of our residents by delivering comprehensive nursing services with professionalism and compassion. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans. Administer medications, injections, IV infusions, and treatments following established protocols while ensuring patient safety. Monitor patients' conditions closely, recognizing signs of deterioration or complications such as acute pain or infectious diseases. Provide wound care, catheterization, tube feeding, and other specialized procedures with attention to aseptic technique and infection control standards. Document all nursing activities accurately using Electronic Health Record (EHR) systems and ensure compliance with HIPAA confidentiality policies. Collaborate with interdisciplinary teams—including physicians, therapists, and social workers—to coordinate discharge planning and ongoing care needs. Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, pain management, and medication administration. Requirements Valid RN or LPN license in good standing; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) are preferred. Demonstrated experience in diverse clinical environments such as hospital medicine, long-term care facilities, or assisted living communities. Proficiency with EMR/EHR systems for accurate medical documentation. Experience working with geriatric patients, individuals with developmental disabilities, dementia or memory care needs is highly desirable. Strong understanding of infection control practices, medication administration protocols, and long-term care regulations. Excellent communication skills with the ability to work effectively within a multidisciplinary team while maintaining compassionate patient interactions.
