Core Responsibilities:

Conduct thorough risk assessments for Medicare Advantage members in their homes (average visit: 45 minutes) Evaluate current and prior medical history, medical conditions, and medications Educate patients regarding their medical conditions and medication usage Identify care gaps and recommend appropriate follow-up or actions Visits may include annual health assessments or post-discharge follow-ups No treatment plan creation or medication prescribing required

Desired Qualifications:

Valid Nurse Practitioner license in the state of Indiana Board Certification through AANP or ANCC Active CPR certification (BLS) MSN degree from an accredited nursing school At least 6 months of recent experience as a Nurse Practitioner is preferred
