Key Responsibilities:
Complete annual and post-discharge home health risk assessments for Medicare Advantage members Review and document detailed past and current medical histories, medications, and chronic conditions Educate patients about their medical conditions and prescribed medications Identify gaps in care and recommend solutions to support continued health and well-being Focus assessments solely on evaluationno requirement to develop treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner license in the state of Indiana Board Certification through AANP or ANCC Current BLS certification (CPR) MSN degree from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner is preferred
