13-week contract with possible extension Full-time, Monday through Friday On-call duties may be required

Key Responsibilities:

Deliver in-home nursing care and skilled health interventions Complete comprehensive patient assessments and OASIS documentation promptly Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans Administer wound care, manage medications, provide patient education, and monitor disease status Communicate all patient updates and changes in condition to the clinical team in a timely manner

Qualifications:

Active RN license Minimum of 1 year recent experience as a home health RN Proficiency in OASIS is essential Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and a valid drivers license Ability to independently manage a structured, full-time schedule with daily patient visits
