Responsibilities:

Complete annual home health risk assessments in patient homes, averaging 45 minutes per visit Assess prior and current medical history, conditions, and medication use Educate members about their health conditions and medication regimens Identify care gaps and recommend steps for resolution No requirement for creating treatment plans or prescribing medication

Desired Qualifications:

Active Nurse Practitioner license in Indiana Board certification through AANP or

ANCC MSN

degree from an accredited nursing school Current BLS (CPR) certification At least 6 months of recent NP experience is preferred Expect competitive pay between $80 and $90 per evaluation, with a mileage stipend and compensation for administrative time included.
